Playing with a Loose table or B not raising enough preflop to know the range of that persons hand. Time and time again you see people at 1/2 tables just calling or only raising $4 when they have rockets to thus get 3-6 other callers and then get pissed when someone runs into a Flush off of 2-3 suited.
