Request for poker log template

For everyone that keeps track of their daily/hourly poker play details, what information do you find necessary to keep note of to improve your game? And how do you arrange your informtion? Do you use a 3rd party program to read through your hand history details to aid you?

I've been wanting to improve my game to a higher level. Any suggestions on how I should do this would be appreciated
