They put their opponents on a range, make an estimate of how often he'd fold to a 3bet there and see if the play is immediately profitable. If it is, it doesn't matter what they're holding. Of course, the crazy hands you see on TV aren't the ones the opponent folds, you see a call and the aggro online player actually has to play that trash post-flop.
