In all 4 situations above the raiser made no verbal declaration.

Answer to 1 - 3 is NO.  Answer to 4 depends.
1.  The one chip rule applies here, even though player already has a small blind in.
2.  Since neither chip can stand alone as a call here, it is not a raise even though it meets the rule that it has to be equal to or greater than previous bet.
3.  One chip rule applies here, easy one.
4.  If 50% rule is in effect (most casinos and some stand alone card rooms) then yes, if not then no and extra chips get returned.[/SPOILER]
