If you read some of the older books on poker, the authors will often mention this when talking about check/raising.   It was more of a house rule, and never an official rule.  The authors advise you to be aware of the house rules, but if there is no rule they say there are perfectly valid spots for it.

These days, it's considered a perfectly valid and normal part of a person's strategy.  And, I doubt anyone would have ever gotten shot in a live game, even where it was against the rules.  I suspect the actual punishment would have been along the lines of refusing the raise (declaring it a call) and a warning.
